Mail in the Completed Form with paymentfor purchase amount, to the address located on the bottom of the form.
Overseas Orders - There is an additional charge for orders not shipped to an address in the Continental US.
Orders Placed By Mail - Mail-In Purchases require a check , money order or credit card for the purchase amount. Mail Order Credit Card Purchases Accepted! No C.O.D orders accepted. Normal shipping is 2 weeks after recept of payment.
